There are four state-regulated forms for our preschoolers. Listed below are brief descriptions of each of these documents, plus links to the most current versions, as maintained by the Ohio Department of Job and Family Services (ODJFS).
1. Child’s Medical Statement (Required) The State of Ohio mandates that MCCP collect and maintain written verifications of annual medical exams for every child enrolled in our facility. State rules require an updated form every year for all children, unless they’re entering kindergarten. 2. Child Enrollment and Health Information (Required) This 2-page form must be completed on or before your child’s first day of school. 3. Child Medical/Physical Care Plan (Optional) Whenever a child has special health needs, this form is used to officially notify preschool staff. If applicable for your child, this form should be provided to MCCP, and then updated annually, or as often as necessary. 4. Request For Administration of Medication (Optional) This form is necessary only if medication(s) will need to be administered while your child is attending preschool. Once completed and submitted to MCCP, any medication form on file is valid for only 12 months, and one form must be used for each medication needing to be administered. Box 1 must be completed by the parent/guardian; a licensed practitioner is responsible for completing Box 2. MCCP is then responsible for recording each administration, of every medication specified, on the backside(s) of the submitted form(s) (Box 3). As a reminder – our children cannot start school without a medical statement and enrollment form on file with MCCP. Please contact Renee if you have any questions or concerns.
